Connecting people to resources, navigating essential community services

Medicaid Targeted Case Management (MTCM) helps eligible individuals and families identify needs, locate available resources, and navigate services that may address challenges affecting daily life. It may be relevant when someone is having difficulty finding housing assistance, food, transportation, healthcare, educational resources, or other community support. Services are available to eligible individuals throughout South Carolina, including those in Ridgeland and surrounding communities.

How It Works

MTCM may assist with identifying and connecting eligible clients to resources involving:

  • Housing and rental assistance
  • Utility and light-bill assistance
  • Food resources and food pantries
  • Clothing and shoes
  • School supplies and educational resources
  • Transportation resources
  • Employment and job-readiness resources
  • Child and family resources
  • Medical and behavioral health providers
  • Community programs
  • Government and benefit programs
  • Financial assistance programs
  • Budgeting and financial-resource information
  • Furniture and household-item resources
  • Hygiene and personal-care products
  • Holiday and seasonal assistance
  • Free community events and programs
  • Free or reduced-cost goods and services
  • Donations and community giveaways
  • Other resources based on the individual’s assessed needs

What to Expect

MTCM looks at the whole person and family rather than focusing on only one concern, helping identify available programs and resources that may address the individual’s assessed needs. A client may receive assistance navigating community programs and service systems and connecting with organizations that may be able to provide appropriate support. Availability and eligibility for financial assistance, free items, donations, and community programs are determined by the organization providing the resource and are not guaranteed by Leads Way Therapeutic Services.
